What to Expect After HoLEP Surgery: Recovery and Aftercare Guide

HoLEP (Holmium Laser Enucleation of the Prostate) is the most effective minimally invasive treatment for an enlarged prostate, but the smoothness of recovery depends a lot on knowing what to expect. This guide walks you through the first 24 hours, the first week, and the first three months after surgery, so you can plan your time off, your activity, and your follow-up with confidence.

The first 24 hours: hospital and catheter

HoLEP is performed under general or spinal anesthesia and usually requires a single overnight stay. You will wake up with a urinary catheter that gently irrigates the bladder to flush out small clots. Discomfort is mild and well controlled with oral pain medication; most patients are walking the same evening.

The irrigation is normally stopped the next morning and the catheter is removed before discharge in most cases. You should be able to urinate on your own immediately, although the stream may feel different at first.

The first week: bleeding and urinary symptoms

It is normal to see some pink or light-red color in the urine on and off for two to three weeks while the prostate bed heals. Episodes can come and go even when everything is fine. Drinking plenty of water (about 2 liters per day) keeps the urine clear and helps the healing scab pass without obstruction.

Urgency, frequency and mild burning when urinating are expected during the first one to two weeks. They are caused by the inner surface of the prostate healing, not by infection, and they improve steadily. A small percentage of patients have temporary, mild urinary leakage that resolves within a few weeks with pelvic-floor exercises.

Activity, work and driving

Most patients return to desk work in 5 to 7 days. Walking is encouraged from day one — it prevents blood clots and helps recovery. Avoid heavy lifting (more than 5 kg), intense exercise, cycling and long car trips for 3 to 4 weeks to protect the healing prostate bed and reduce the risk of delayed bleeding.

Driving is usually fine once you no longer need narcotic pain medication and can react comfortably — typically after a few days.

Sexual activity and ejaculation

Sexual activity can be resumed at around 3 to 4 weeks, once bleeding has fully stopped. HoLEP preserves erectile function: it does not damage the nerves responsible for erections.

What does change in most patients is ejaculation: semen travels back into the bladder instead of out (retrograde ejaculation). The sensation of orgasm is preserved, and the semen is simply expelled later with the urine. It is harmless but does affect natural fertility.

Follow-up and long-term results

A follow-up visit is scheduled around 4 to 6 weeks after surgery to confirm healing and review urinary flow. International patients are followed by telehealth in English. By the third month, the urinary stream is strong, nighttime trips to the bathroom decrease dramatically, and most patients no longer need prostate medication.

HoLEP results are durable: because the obstructing tissue is fully removed, recurrence is rare and the improvement typically lasts a lifetime.

Warning signs: when to call your urologist

Contact the team if you see bright-red urine with large clots, are unable to urinate, develop fever above 38°C (100.4°F), severe pain not controlled by medication, or notice worsening burning that lasts beyond two weeks. These are uncommon but treatable when caught early.
